PASSENGER DETAILS:
Fill in the passport details below. This is required for booking flights, trains, Inca trail permits, Machu Picchu entrances, and hotels etc.
NOTE: We require a scanned copy or photo of the photo page of your passport to make all of your bookings- this is especially important for Inca Trail permits. You can attach it below or email it to us later at: [email protected]. Peru is strict on passport names and numbers. Please record it exactly as written in your passport and you must let us know if there is any change.
A passport change could invalidate your reservation and you may be refused to travel.

By filling in this form you understand that travel medical insurance covering personal accident, emergency evacuation, and repatriation expenses is not included on any Amazonas Explorer tours, and that you will possess such insurance throughout the dates of the above-listed trip, and/or you are responsible for your own personal medical expenses.
You agree that you are responsible, and/or will possess travel medical insurance, or your insurance plan will cover you while abroad throughout your Amazonas Explorer adventure.
Participation Statement: By booking this trip I/We understand that the activities that we are booking carry inherent risks and risk of accident and/or injuries. We also understand that we are empowered to ask questions as to the nature of these risks.
After we receive the form we will be in contact with you with an invoice. The invoice will share the Price per Person, Total Price, and the Non-Refundable Deposit, all listed in USD. Your trip will be confirmed after you have paid the deposit.
